Standard Video Taping Agreement

- Janet E. Lapp, Ph.D., Inc., (Performer) grants to ____________________________
(Client), a nonexclusive, nontransferable, non-sub-licensable
license and right to make a video recording (Video Master)
of Performer's presentation at the above identified program
(Presentation) for archival use only. If said video shall
be used for any other purpose, including resale or training,
a separate licensing agreement shall be made.
- Client hereby acknowledges and agrees that Performer shall
be and is the owner of all rights, title and interest in
and to the Presentation, Presentation material and the Video
Master, including the copyright, and that Client shall affix
the following notice and statement to the Tape label and
in the opening billboard of the Tape:

No portion of this tape may be reproduced by any means without
written permission.
- Client further agrees to provide Performer at Client's
sole expense:
- The original Video Master of the Presentation or the
edited Video Master, if editing is done to produce the
Tape, no later than thirty (30) days following the Presentation..
- One (1) VHS copy of the Video Master no later than
one (1) week following the Presentation;
- Client shall deliver items a and b as defined above,
by insured carrier at Client's expense to:
